PO Box 9940 Mobile, AL 36691-0940 If you have found a misspelling of your name, please call us at 1-866-876-3018. Please note there are different requirements for different types of name changes. LEGAL NAME CHANGE We will need to see legal documentation of your name change before adjusting our records (copy of marriage certificate, divorce decree, or other court order). Please send your marriage certificate or recorded divorce decree or court order to the address listed above, and your request will be processed upon receipt. CHANGE OF OFFICERS - BUSINESS ACCOUNTS Please provide Mitsubishi with a copy of the minutes of the corporate meeting installing the new officers as authorized signers on behalf of the company. We will complete your request upon receipt of your supporting documents.
